What Is Case Management?

Case management is personalized support that helps you get the care and resources you need, both inside and outside the clinic. HSNT’s case managers look at your medical, social, and financial needs to create a personalized plan that works for you.

Every person’s situation is different. That’s why our team listens first and builds a plan based on your goals, challenges, and priorities. Our role is to help remove barriers so you can focus on your health and your family.

How Case Managers Can Help

HSNT can connect you to community and health-related resources, including:

  • Transportation to medical appointments
  • Dental and vision care referrals
  • Guidance on health insurance
  • Medication and prescription assistance
  • Support with medical testing

Our case managers are experienced in navigating complex systems and advocating for patients who need extra support.

Who Qualifies for Case Management?

Case management services are available by referral from your HSNT provider. If your provider feels you could benefit from extra support, our case management team will meet with you to review your needs and recommend next steps.

You may qualify if you need help in one or more areas, such as:

  • Managing medications
  • Accessing care or transportation
  • Understanding insurance or benefits
  • Coordinating medical services

HSNT offers case management at our Denton and Plano locations.

Case Management FAQs

Healthcare can be confusing and hard to navigate. A case manager helps you understand your options, complete paperwork, find the right services, and communicate with care providers and community agencies. Case managers also help with transportation, health education, and finding financial assistance when needed.

Case management usually includes four main steps: Intake: We gather information, identify urgent needs, and get to know you Needs Assessment: We identify challenges, set goals, and discuss next steps Service Planning: We create a plan, set timelines, and track progress Monitoring and Evaluation: We review outcomes and adjust support as needed
